Welcome to Pediatric Providers. We are happy to have you join our practice and look forward to working with you to keep your children healthy.

New patient appointments take more time than established patients. Because of this, we will have to cancel the appointment if the paperwork is not received prior to the appointment and missed new patient appointments will not be rescheduled.

Becoming a Patient with Us

1. Make an Appointment: Call us to schedule an appointment. When you call, we will ask for some basic information and set you up with the patient portal​

2. Complete Registration Forms: Sign onto the patient portal and complete the forms required - this includes a HIPAA Privacy and Practice Policies, Patient Registration Form, and Patient History Form

3. Ensure All Personal Information is Accurate: Check the Personal Information section and ensure Basic Details, Emergency Contact, and Insurance information are all correct. If anything is incorrect, please message the office in the portal or call us.

4. Medical Records: Complete and send the Medical Records Release Form. You can either email or fax it to us so we can ensure we have your medical records before your first visit. We require a minimum of immunization records to keep your first appointment.

Medical Records

Prior to your appointment you can authorize for us to request your medical records by signing a Medical Record Release form. Or you can request your child/children medical records from your previous pediatrician and submit to our office before your appointment.

Copays

Please be prepared to pay the copay as required by your Insurance
