Welcome to Pediatric Providers. We are happy to have you join our practice and look forward to working with you to keep your children healthy.

 

Please complete the forms that are part of the New Patient Package AND send them to the patient portal prior to your appointment. We will then print the paperwork that needs your signature at the visit. Please make sure that we have received patient's immunization record prior to the visit. 

Because of the time required for a new patient physical appointment, we will have to cancel the appointment if the paperwork is not received prior to the appointment and missed new patient appointments will not be rescheduled.

Medical Records

Previous to your appointment you can authorize for us to request your medical records by signing a Medical Record Release form. Or you can request your child/children medical records from your previous pediatrician and submit to our office before your appointment.

Copays

Please be prepared to pay the copay as required by your Insurance
